Pediatric Parenteral Nutrition Solution (PAS) Order Form

Dextrose Concentration: 8-12 % peripheral
12 - 30 % central
Fats: Start with 0.5 - 1 g/kg/day
Advance 0.5 - 1 g/kg/day up to 3 g/kg/day

% of total calories in fat should be less than 40 %
Amino Acids:

In children under 2 years of age use Trophamine

Start with 1 g/kg/day
Advance by 1 g/kg/day (Max = 3 g/kg/day)

% of total calories in protein should be 6 - 8 % in renal or hepatic failure
% of total calories in protein should be 12 - 17% in others

Electrolytes: Na: 2 - 4 mEq/kg/day
K: 2 - 3 mEq/kg/day
Mg: 0.25 - 1 mEq/kg/day
Heparin: Recommended for central line use only: 0.25 - 1 Units/ml
Trace Elements:

Regular Trace Elements contain Chromium, Copper, Manganese, Selenium and Zinc.
If you choose not to add Trace Elements, consider adding back Chromium, Selenium and Zinc.

Albumin: 1 g/kg/day
Ranitidine: 2 - 6 mg/kg/day
Vitamin K: Additional Vitamin K is only needed in specific cases such as a bleeding disorder.
Dose: 2.5 - 5.0 mg/day
Carnitine: Neonates: 10 - 20 mg/kg/day (max: 50 mg/kg/day)
